How Floodwaters Can Compromise Public Health

Floodwaters can transform vibrant communities into perilous environments in the blink of an eye. As climate change intensifies storm patterns and precipitation, the reality of flooding becomes a chronic threat to public health. In fact, on the 27th of September 2024, Hurricane Helene made landfall in Florida and decimated areas from Florida right up to Western North Carolina and beyond. However, the devastation it left behind in North Carolina will be felt for generations to come as towns and communities were totally wiped out. It was the most catastrophic storm in over 100 years of history.

Understanding the ways floodwaters can endanger the well-being of residents is crucial for an effective response.

The risks associated with floodwaters extend far beyond their immediate physical dangers, presenting complex challenges such as waterborne diseases and the release of harmful chemicals. Contaminated water can promote the spread of illnesses such as Legionella and Pseudomonas, among others, while pollutants from industrial sites can seep into the floodwaters. This exacerbates the threat to public health. There is a lack of education when it comes to understanding these threats. Therefore, awareness of these potential dangers is essential for communities and organizations to navigate the aftermath of flooding. North Carolina is already seeing a spike in potential waterborne illness as well as respiratory problems. 

By understanding the risks and proactively addressing them, communities can build resilience against the ever-present threat of flooding. Furthermore, organizations and companies can be more proactive in protecting their communities.

The Impact of Floodwaters on Public Health

The aftermath of a storm can bring significant environmental health risks, particularly through toxic water. Heavy rainfall from a hurricane like Helene leads to swift water accumulation that often breaches sewage systems, industrial facilities, and agricultural sites, turning benign waterways into reservoirs of toxic chemicals and water pathogens.

The water becomes unsafe to drink, even after boiling. Drinking water safety becomes critical as these contaminants enter clean water supplies. As a consequence, using pressure washers, showers and other systems such as hoses and water outlets can spread pathogens such as Legionella and Pseudomonas. 

Infectious diseases from organisms such as Legionella, E. coli, Pseudomonas, and Hepatitis A virus thrive in these conditions, increasing the risk of infection. 

Pathogens in Floodwaters

Here is a list of human pathogens that frequently contaminate local drinking water sources (wells and municipal water systems).

  • Legionella (causes Legionnaires' disease)

  • E. coli (causes gastrointestinal illness and systemic infections)

  • Hepatitis A virus (Liver infection)

  • Vibrio Cholera (causes watery diarrhea, vomiting, leg cramps, restlessness, irritability, dehydration, kidney failure, shock, coma, and death)

Further threats exist from facilities such as chemical and fertilizer mining and large or small-scale fuel storage locations. This is because flood waters often come into contact with these hazardous materials and transport them in flood waters impacting local areas and beyond. 

Post-storm, contact with floodwater, especially near downed power lines or after chemical spills, should be avoided to reduce the risk of electrocution. Risk management plans must address environmental risks and establish protocols for the safe return to the affected areas. The priority is always human health, ensuring clean water is accessible and the public is aware of the continuing risks.

Understanding the Risks Associated with Floodwaters

Floodwaters are like moving targets; the water harbors a plethora of environmental contaminants that can wreak havoc on human health. Overflowing sewage systems spill waste material into the streets, which harbor dangerous bacteria.

 Agricultural runoff cascades a cocktail of pesticides and animal waste through communities. Chemical spills can contaminate standing water, and pests can thrive in this environment, adding another potential risk for disease. These exposures are not merely inconvenient; they represent serious health risks, from gastrointestinal illnesses to respiratory problems when sewage becomes airborne and dries out.

Legionella, a particularly nasty bacteria, can spread through the inhalation of water droplets. It poses a very real threat after the storm has passed and the waters begin to stagnate. Even long after the storm, legionella is a potential bomb waiting to explode.

Toxic Chemical Releases

As an example of what is discussed, Hurricane Ian's fury in 2022 resulted in the unimaginable—millions of gallons of hazardous wastewater spilled into otherwise pristine wetlands. It's a recurring nightmare in the wake of hurricanes, exemplified by Hurricane Ida's shocking aftermath, with over 2,000 chemical spills reported. Now Hurricane Helene has left even more devastation, and the threat of increased pathogens has exploded. 

Households are not exempt from contributing to the toxic slurry, with common items like bleach and medical waste becoming part of the mix. Sewage is yet another factor that significantly escalates the risk of encountering pathogenic substances.

Industrial Sites and Petrochemical Complexes

The proximity of industrial sites and petrochemical complexes to communities is of particular concern. There is a risk that cooling systems at chemical facilities may fail, leading to catastrophic fires and the release of pollutants into both the air and water. This multifaceted pollution not only impacts immediate air quality but also carries the latent danger of settling into communities, presenting long-term health risks.

Industrial materials cause contamination with no warning, and yet residents are unaware of the toxic dangers that are now immediately present in the environment. 

Communities that reside near these hotspots are at an elevated risk and require protective measures such as early warnings, evacuation plans, and post-event cleanup and remediation strategies to guard against the full spectrum of possible toxic exposures. The collaboration between industrial facilities, government agencies, and local organizations is imperative to formulating such protective strategies.

To ensure better preparedness and rapid response in the event of a storm-induced toxic water crisis, it's vital that critical data from research is integrated into the operational plans of facilities, especially those located within identified hazardous flood-prone areas.

These systematic strategies, when effectively implemented, can significantly mitigate environmental risks to human health and safeguard communities from the countless hidden dangers presented by toxic floodwaters.

Significance of Community Awareness

Community awareness plays a pivotal role in safeguarding public health, especially in the aftermath of a storm. Given the unpredictable nature of flooding and the potential for waterborne pathogens to cause illness, communities are urged to proceed under the assumption that all flood waters are contaminated.

Enhanced awareness and education on flood safety measures are not only helpful but vital elements in protecting public health. Residents must be promptly informed about the condition of their water through measures such as Boil Water Advisories to maintain safety in their drinking water. In times of disaster, the role of local reporting becomes indispensable in ensuring that communities remain aware of potential contamination risks associated with flooding. Clear and timely communication from public water systems during and after hurricanes is essential for informing residents whether the water is safe for use or not.

There is a heightened significance for local health departments to monitor and report on the quality of water and the risks to public health. These departments should be diligent in identifying and communicating any potential threats to human health arising from floodwater contamination, including chemical spills and the introduction of chemicals into the water from industrial facilities.

Preventive Measures and Precautions

When a storm leaves behind flood waters teeming with potential hazards, it's crucial that communities and individuals take proactive steps to safeguard health and prevent illness. A first line of defense is to avoid direct contact with floodwater as much as possible. If contact is unavoidable, protective clothing and waterproof bandages over open wounds are essential to reduce the risk of infection. If you have been exposed to flood waters, change clothes and clean all exposed skin to remove contaminants that are likely to be in the water as soon as possible.

Moreover, any wounds that have been in contact with floodwater need to be monitored closely. Seek medical care for wounds, even seemingly small cuts, that look or feel infected or if you experience symptoms such as fever, respiratory illness, or confusion. These signs warrant immediate medical attention to prevent further health complications.

Testing Water Quality

After floodwater recedes, attention must turn toward water quality, especially for those who use well water, as contamination from bacteria and chemicals is EXPECTED TO OCCUR. Residents who depend on private wells should have their water tested to ensure it's free from toxic chemicals and water pathogens. In the meantime, while samples are being collected and laboratory analyses performed, well-water users should ASSUME CONTAMINANTS ARE PRESENT  and use appropriate precautions to avoid becoming ill.  It's important to remember that the purity of well water can fluctuate greatly after heavy rainfall or flooding, emphasizing the need for regular and continuous monitoring. 

Even residents on public water systems aren't exempt from vigilance. Local municipalities are responsible for testing and treating municipal water, but household plumbing systems may introduce new contaminants or be affected by damages from the storm. Therefore, testing city water is absolutely necessary to identify hazardous conditions early and minimize the threats to human health.

Considering the potential for water system contamination, residents should evaluate the benefits of having an on-site point of entry or point of use water purification system to mitigate the effects of storm-related water contamination and ensure a quicker transition to safe water consumption post-storm.

Recognizing Symptoms of Illness

Awareness of symptoms associated with contaminated water consumption can be life-saving. Initially, signs of illness may resemble the flu, such as gastrointestinal distress, headaches, or muscle aches. However, if symptoms like confusion, severe digestion issues, fever, or respiratory concerns follow interaction with floodwater, it is critical to seek medical aid promptly. It is imperative to understand the threat associated with contamination from pathogens in the air, such as legionella, which can also resemble the flu but have drastic consequences to health.

Predominantly, it's those who may be immunocompromised individuals, the elderly, and young children—who should be carefully observed.  waterborne diseases may escalate quickly to severe and life-threatening infections within these demographics. 

Building Resilience in the Face of Flooding

In the face of increasingly frequent and severe flooding, particularly in regions laden with industrial facilities, building resilience is crucial for protecting both environmental and human health. The Gulf Coast, a known hotspot for such facilities, often sees relaxed pollution restrictions in emergencies, exacerbating chemical hazards in flood events. Delayed incident notifications from the National Response Center further impair public safety during these critical times.

Communities are urged to proactively map flood-prone areas, particularly those housing vulnerable infrastructures likely to release toxic materials, to enhance risk management plans. 

These practices, along with vigilance around downed power lines and awareness of chemical spills, are essential steps in fostering a resilient response to the intricate challenges posed by flood-induced contamination events.
